The last time we vacationed with my dog, the hotel asked us for an accommodation request. Where can I get one?

A request for an accommodation is a specific document required by the Americans with Disabilities Act. It is a request made by a person according to his or her unique needs, and it is mostly required when it comes to the living conditions of someone with a service animal. I didn't know you had to get some kind of request and that they wouldn't let you live with your dog without one. Even if she is emotionally supportive. Thank you for sharing.
